Medical - Minnesota
Provider Blue Cross and Blue Shield of Minnesota - Preferred Gold (Blue Plus Network) and Aware PPO
Plan Year October 1 - September 30
Open Enrollment occurs in September.
Summary Deductible: $300 per person ($600 family max.) in-network, $500 per person ($1000 family max.) out-of-network per calendar year.
Rate of Benefit:
Hospital: 80% coverage in-network, 60/40% out-of-network.
Office Visits: $20 co-pay
Prescription Drug Plan: 80% coverage with a $10 min. & $25 max. co-pay per prescription 31 day supply, must be on approved drug list with a network pharmacy. Generic mandate. Mail order prescription plan available.
Eligibility Any regular employee who is scheduled to work 30 or more hours per week is eligible on the first day of the month following 30 days of employment.

The pre-tax costs per biweekly paycheck are:

Dental
Provider Delta Dental (PPO or Premier Provider Network) – Base and Buy-up Plans
Plan Year October 1 - September 30
Open Enrollment occurs in September. Late Entrant Provisions apply without a Qualifying Event.
Summary Deductible: $0 for diagnostic/preventive or orthodontic procedures.
All other procedures: $25/calendar year.
Rate of Benefit:
Diagnostic/Preventive: 100% (e.g., cleanings)
Basic Procedures: 80% (e.g., fillings)
Major Procedures: 50% (e.g., crowns)
Orthodontic Procedures – Base Plan: 50% (e.g., braces) – dependent children only; Lifetime Maximum Benefit: $1,000
Orthodontic Procedures – Buy-up Plan: 50% (e.g., braces) – adults and dependent children; Lifetime Maximum Benefit: $1,500
Calendar Benefit Maximum – Base Plan: $1,000
Calendar Benefit Maximum – Buy-up Plan: $1,500
Eligibility Any regular employee who is scheduled to work 30 or more hours per week is eligible on the first day of the month following 30 days of employment.
Cost

Flexible Spending Account (FSA)
Summary Employees may set aside pre-taxed dollars to pay for dependent care and/or health care expenses (e.g., deductibles, co-pays, eyeglasses). Direct deposit is available.
Eligibility Any regular employee who is scheduled to work 30 or more hours per week is eligible on the first day of the month following 30 days of employment.
Cost No cost to enroll. In fact, there are cost (tax) savings and advantages to using this benefit.

Life and AD&D Insurance
Provider Lincoln Financial
Summary Life insurance benefit of 3x salary up to $600,000. Accidental Death and Dismemberment (AD&D) benefit of 3x salary up to $600,000. Additionally, NMDP provides life insurance of $10,000 for an eligible employee's spouse; and $5,000 for a dependent child from age 6 months through 18 years or to age 23 if child is a full-time college student.
Eligibility Any regular employee who is scheduled to work 30 or more hours per week is eligible on the first day of the month following 30 days of employment.
Cost This is a company-sponsored benefit. Employees are taxed on premiums paid for insurance over $50,000.

Travel Accident Insurance
Summary All active employees, including couriers, working 20 or more hours per week are provided $500,000 in supplementary accidental death and dismemberment coverage while on approved business trips or while doing business for the NMDP.

Voluntary Life and AD&D Insurance
Provider Lincoln Financial
Summary Employees may choose life insurance coverage only or life insurance with AD&D coverage over and above that already provided by the NMDP. Insurance benefits may be purchased in $10,000 increments up to 7x the employee's annual base salary, not to exceed $300,000. Additionally, coverage may be purchased for an employee's spouse in $10,000 increments up to 50% of the employee's eligible amount, not to exceed $150,000. Coverage for dependent children ($10,000) may also be purchased if an employee or their spouse elects coverage for themselves.
Eligibility Any regular employee who is scheduled to work 30 or more hours per week is eligible on the first day of the month following 30 days of employment, subject to underwriting approval.
Cost Cost is dependent on age and amount of coverage.

Short and Long Term Disability
Provider Lincoln Financial
Summary These plans provide partial income replacement benefits for medical disability that results in extended employee absences. Short Term Disability (STD) - 12 weeks of coverage at 70% of eligible employee weekly salary.
Long Term Disability (LTD) - Begins after the 91st day and can continue until normal retirement age if certain criteria is met. Payable at 60% of monthly salary to a max. of $10,000/month. LTD payments are tax free.
Eligibility Any regular employee who is scheduled to work 30 or more hours per week is eligible on the first day of the month following 30 days of employment.
Cost This is a company sponsored benefit at no cost to employees.

403(b)
Summary A custodial account made up of mutual funds that has tax deferred savings for retirement through biweekly payroll deductions. This is a voluntary contribution, however, employees are automatically enrolled unless they instruct otherwise.
Eligibility Any employee can begin contributingon the first day of the month following 30 days of employment.
Cost NMDP will automatically deposit your elected payroll amount.

Pension
Plan Year Oct. 1 - Sept. 30.
Summary NMDP contributes 6% of an eligible employee's fiscal year wages, plus 5.7% of wages in excess of maximum social security taxable wage base for the year. A contribution is made for employees after September 30 following 500 hours worked.
Vesting schedule: 2 years = 40%, 3 years = 100%
Eligibility Any employee who is at least 20 -1/2 years of age enters the plan on Oct. 1st following their hire date.
Cost This is a company sponsored benefit at no cost to employees.

Compensated Time Off (CTO)
Summary Full-time employees accrue 18 days per year for the first two years. One additional day accrued every two years, up to 25 days. Must use CTO time before unpaid time can be taken. CTO must be accrued before it is used. Accrual begins immediately upon hire. Employees who are scheduled to work fewer than 37.5 hours per week will accrue CTO on a pro-rated basis.
Eligibility Any regular employee who is scheduled to work 20 or more hours per week is eligible on their first day of employment.
Cost This is a company sponsored benefit at no cost to employees.

10 Paid Holidays
Summary As designated each year by NMDP. Includes a Floating Holiday, New Year's Day, Martin Luther King Jr.'s Birthday, Memorial Day, Independence Day, Labor Day, Thanksgiving Day, Day after Thanksgiving, Christmas Eve Day, and Christmas Day.
Eligibility Any regular employee who is scheduled to work 20 or more hours per week is eligible on their first day of employment. (Part-time is pro-rated.)
Cost This is a company sponsored benefit at no cost to employees.

Educational Assistance
Summary NMDP pays for professional development activities such as conferences, seminars and conventions that are job related as well as tuition for undergraduate and graduate courses or degrees. $2,000 fiscal year maximum.
Eligibility Any regular employee who is scheduled to work 20 or more hours per week is eligible for Professional Development Activities on their first day of employment and Tuition Reimbursement after 6 months of employment.
Cost This is a company sponsored benefit at no cost to employees. Approval and payment is based on available funds for the fiscal year.

Summary Confidential counseling available via telephone to assist employees and their families with issues of daily living. Topic areas include: Personal, Family, Work, Legal, Financial, Alcohol/Drugs and Mental Health.
Eligibility Open to all employees and immediate families at any time.
Cost This is a company sponsored benefit at no cost to employees.

Summary NMDP directly deposits your paycheck into your checking/savings account. A voided check and/or deposit information is required.
